Before beginning to practice with any team, a student-athlete must have a physical if a new candidate or a health screening if a returning athlete. Because the university does not carry health insurance for its athletes, all candidates must provide proof of insurance through a personal policy or their parents' policy. A varsity candidate must also attend a mandatory meeting at which forms will be completed and rules and regulations discussed.

Physical Form (Freshman and First Year athletes ONLY)
According to NCAA Bylaw 17.1.5: All student-athletes who are beginning their initial season of eligibility shall be required to undergo a medical examination or evaluation administered or supervised by a physician. This examination must be administered within six months prior to participation in any practice, competition or out-of-season conditioning activities. Acceptable physicals must be completed by a Licensed Medical Doctor (MD), Doctor of Osteopathy (DO) or Physicians Assistant (PA). Physical forms submitted utilizing other practitioners will NOT be accepted.
